Abstract Introduction Soft tissue leiomyosarcomas represents 0.7% of all malignant tumors. The most common location is the retroperitoneum, and appearance local or distant recurrences after resection not uncommon. Pathological study important to establish confirmatory diagnosis prognosis this type lesion. Clinical case We present clinical a patient with retroperitoneal leiomyosarcoma that was resected associated ipsilateral nephrectomy. An 11-month follow-up revealed hepatic lesion in segment 5, suggestive metastasis. decided on surgical Liver segmentectomy performed by laparoscopic approach. pathological liver showed tumor about 5 centimeters, compatible metastasis, presenting abundant mitoses (8/10), 15% necrosis central area Ki67 20%. Discussion Leiomyosarcoma very aggressive tumor.
